If you would like to share a concern with SWT Leadership please complete the following form, either anonymously or with your contact information for follow up. This form can be completed by clients, guardians, referrers, or other stakeholders. All reports are reviewed by SWT Leadership and responded to on a case-by-case basis

South Carolina Student Referral Form

If you would like to refer a student for services, please complete this form. Not every child that is referred will enter care, as guardian consent is required. Please ensure you have all student information ready prior to completing a referral (name, address, DOB, etc.) to expedite the process of enrollment into care. Only students in South Carolina are eligible for these services.
